PURPOSE
A Good Faith Estimate is as estimate of the expected cost of services based on the client's individual needs and the therapist's professional judgment. It is important to note that a good faith estimate is no a guarantee of the final cost of services, as the actual cost may vary depending on a number of factors, including the length of treatment, the complexity of the client's case, and any changes to the client's treatment plan.
PROCEDURE
The therapist will provide the client with a good faith estimate during the second therapy session upon request by the client. It will include the following information:
UPDATES
The therapist will update the good faith estimate as needed throughout the course of treatment. For example, if the client's treatment plan changes or the client needs additional sessions, the therapist will update the good faith estimate to reflect these changes.
PAYMENT
The client is responsible for paying for all therapy services rendered, regardless of whether the client has insurance. The therapist will accept payment in cash or credit card. The client is responsible for paying any co-pays, deductibles, or coinsurances that are not covered by their insurance.
QUESTIONS
If the client has any questions about their good faith estimate or their financial responsibility for therapy services, they should speak to their therapist directly.
